How do I Make an Appointment?

You can easily make an appointment by calling us at (312)-588-1104 during office hours OR simply click on the icon Make an Appointment to your right. This will take you to our patient portal where you will be able to see open time slots for appointments. Click on one, and fill in the corresponding fields to easily schedule an appointment at your convenience.

After registering as a new patient, please ask our staff to assist you with setting up your online patient portal account. This online connection will give you access to your patient records, prescription requests, and allow you to contact our physicians and make future appointments. To locate it upon your return, simple click on Patient Portal atop our menu bar.

What Insurance do you accept?

We are always adding new insurance carriers to our clinic. At present, we accept the following:

What Personal Medical Information should I Bring with me for my Appointment?

For all appointments, we ask that you bring your insurance card and a photo ID.

You should also bring a list of all of your prescription and over-the-counter medications with you at each visit. This helps us to stay abreast of any changes in your personal medical information and better ensure your overall care.

If you are a new patient, please request your previous medical records from your doctor(s) and bring them along with you. You may also have them sent to us via U.S. mail or electronically at contact@ASAPWellnessCenter.com

As a New Patient, what should I expect for my First Medical Visit?

Thank you for choosing ASAP Wellness Center for your health care needs!

During your first medical visit to our clinic, you will meet our administrative staff at the reception desk to complete a few registration forms and be provided a copy of our HIPAA disclosure. You will also be asked to complete a health questionnaire for the wellness visit. You will then be ready to see your physician who will review the health questionnaire with you and address all your concerns. So, feel free to bring questions or a supporting family member.

As family physicians, we will try to solve your current medical problem and detect or prevent other health problems.

We hope to make the first visit not just an opportunity to deal with any medical concerns you may have, but also a time to get acquainted with you.

Do you take Walk-in Patient Appointments?

Yes, we do! Because your medical concerns are our top priority, we accept walk-in patients without appointments or same day appointments.

What is Oriental Medicine or Acupuncture all about?

Oriental Medicine and Acupuncture may seem like a mystery to you.

How do I Cancel my Appointment if something comes up?

We know things come up. So, you may wonder, “How do I cancel my appointment?”

You can easily call our office number (312) 588-1104.

Our staff is happy to assist you reschedule your appointment to first available date and time that’s convenient for you.

Please try and let us know at least 24 hours before your appointment, so that we may open up that time to treat another patient.

Why am I getting a bill for Laboratory test from Quest?

At ASAP Wellness Center, our physicians may order additional laboratory tests to be done to assist in your diagnosis and treatment. While many of these are draw or performed on site, we utilize an off site laboratory service company called, Quest Diagnostics.
